Community Resource Worker (CRW1)

POSITION SUMMARY:

The Community Resource Worker (CRW1) acts in a support and assistance role to provide direct support to individuals both within their home and the community based on the principles or Person Centered planning.  The individual will work a variety of shifts. 

DUTIES AND RESPONSIBILITIES:

             Under the direction of the Team Leader and or Residential Coordinator the Community Resource Worker will:

 

  • Provide quality, individualized support to clients in all areas including, but not limited to personal health care, food, clothing and safety while promoting client independence.

  •  Ensure a safe, clean and comfortable living environment for all clients.

  • Know and implement clients Individual Service Plans (ISP’s)

  • Support clients in their daily routines related to employment, volunteer placements and skill developmental.

  • Implement all personal programs and teaching guidelines as required.

  • Encourage and support natural supports.

  • Assist clients to ensure household tasks are completed (house chores, grocery shopping, yare maintenance, laundry)

  •  Exhibit attributes of positive attitude and foster an environment of cooperation and determination.

  • Exhibit professional attitudes and performance on a consistent basis.

  • Be aware and comply with policies and procedures of S.C.O.P.E.

  • Communicate effectively with clients, parents/guardians, community members, other staff and supervisors.

  • Administer medications and PRN’s

  • Daily documentation in client logs and ISP’s.

  • Attend staff meetings and scheduled training and meetings.

  • Ensure all clients funds are carefully documented and accounted for in a responsible manner.

  • Be at work in accordance with the work schedule unless otherwise approved by the immediate supervisor

  • Maintains personal and professional competencies

  • Ensure safety needs for the client are maintained.

  • Ensure all required information is exchanged between staff members during shift exchange

  • Comply with SCOPE’s Restrictive Procedures guidelines and process, at all times

  • Represent self and S.C.O.P.E in a professional manner at all times

  •  Other duties as identified by the Team Leader or Executive Director

 SKILLS REQUIRED / PREFERRED: 

Education:

  • High-school diploma

  • Certification in Standard First Aid and CPR

  • Criminal Record Check
  • Family Intervention Check
  • Medication Administration
  • Positive practices and restrictive procedures
  • Abuse Prevention and Response

 

Experience:

  • Direct experience working with adults with developmental disAbilities

  • Basic Computer Skills

  • Effective oral and written communication skills
  • Physical stamina to meet specific residential job requirements
  • Ability to work independently and in a team environment
  • Good Planning and organizing skills

Team Leader

POSITION SUMMARY: 

The Team Leader: provides direction, support and coordination of services for staff, clients and families, including: planning, prioritizing and monitoring staff activities to ensure provision of quality services; participating in the development, implementation and monitoring of Individual Service Plans; providing support and training for staff; facilitating staff communication; ensuring procedures are followed in regard to specific activities, such as placement, training, leisure, etc.; identifying and analyzing issues of concern and assessing the need for intervention; maintaining appropriate documentation and records; communicating appropriately with clients, families, guardians, staff and community representatives. 

The Team Leader position is a supervisory position; therefore, the individual works independently, receiving minimal supervision and guidance. 

DUTIES AND RESPONSIBILITIES:

             Under the direction of the Executive Director, the Team Leader will:

 

  • Provide direct care and service to clients as require

  •  Ensure clients needs are assessed and advocated on their behalf

  •  Comply with SCOPE’s Restrictive Procedures guidelines and process

  •  Ensure the development and implementation of Individual Service Plans and Individual Program Plans for clients.

  •  Ensure all client programs are being followed and appropriate documentation is completed

  • Ensure the preparation of documentation for annual case conference meetings, and attend case conferences.

  • Establish and maintain open, timely and appropriate communications and relationships with client, families/guardians, staff and the community

  • Actively participate in the selection and orientation of front line staff and volunteers

  • Monitor medication ordering and storage in compliance with organizations standards and ensure proper delivery of client medications

  • Develop and implement staffing model subject to Executive Director’s approval                

  • Ensure appropriate staff coverage

  •  Monitor the activities of the team to ensure the provision of quality services to clients

  •  Facilitate team communication and ensure team members are involved in the decision making process.

  • Provide direction, training and support for staff through coaching and monitoring

  • Ensure procedures are being followed in regard to specific client activities such as placement, training, leisure activities, personal services, etc.

  • Train, supervise and evaluate front line staff

  • Identify and analyze areas of concern and assesses the need for intervention

  • Provide ongoing updates to the Executive Director

  • Be aware of and comply with Policies and Procedures and recommends changes to them

  • Facilitates monthly staff meetings

  • Attends monthly Supervisor meetings

  • Maintains professional currency in the rehabilitation field

  • On call responsibilities as outlined in the on call procedure binder

  •  May be required to work a variety of shifts if needed

  • Other duties as identified by the Executive Director

 

SKILLS REQUIRED

Education: 

  • Rehabilitation Practitioner diploma or University degree or College diploma in Human Services.

  • Standard First Aid and CPR

  • Positive practices and restrictive procedures

  • Abuse Prevention and Response

  • Behavioral training

 

Experience: 

  • Minimum 4 years of direct experience in rehabilitation services, human services or specific experience working with people who have developmental disabilities

  • Supervising a minimum of 4 staff

  • Managing a number of projects at one time

  • Proven leadership abilities in interpersonal communication

  • Observation and assessment skills

  • Working unsupervised

  • Facilitating group discussions

  • Teaching or leading an individual or group
